The issue of malfunctioning slot machines is a fairly recent phenomenon. The reason for the malfunctions relates to the complexity of the software in the slot machine. Prior to the 1960s, the old-fashioned style slot machines would use randomly spinning drums which were pretty much impossible to malfunction and the maximum payout was no more than 100x the bet. The early electro-mechanical machines in the 1960s and 1970s were unsophisticated with low maximum payouts. So even if one did malfunction it probably would go unnoticed as slots offered a fairly small return.
